What information does an EHR contain?
An electronic health record (EHR) is a digital version of someone’s physical paper chart. This record contains all of the information that has been collected in a provider’s office, including:

- Diagnoses
- Medications
- Treatment plans
- Known allergies
- Immunizations
- Laboratory results
- Test results
- Diagnostic imaging
